Physicals 2025-2026
The Pomona Valley Hospital/Medical Center is once again generous enough to offer Physicals for the 2025-2026 school year. They will be held on Wednesday, May 14, 2025 from 4:45-8:00 pm. Please schedule an appointment with Mr. Snapp, no walk-ins without appointments will be allowed. Please see the flyer below.
If you are getting you physical examination at different location. You can download the Claremont Physical Form here. Please make sure that the Doctor includes their office stamp.
